Clinical Care Coordinator
Introduction

A vacancy exists for a Care Coordinator based at Life Kingsbury Hospital, reporting to the Patient Services Manager. The Care Coordinator will play a vital role in facilitating holistic patient care by integrating currently fragmented aspects of orthopaedic, stroke, renal, and other complex medical and surgical journeys. The focus will be on improving patient care outcomes by supporting Unit Managers and the Multidisciplinary Team (MDT) in care activities, ensuring seamless transitions and coordination across the patient journey.

Critical Outputs

Key Responsibilities:

  1. Patient Management & Care Coordination
  • Conduct health assessments to identify patient risk factors.
  • Develop and implement pre-defined interventions to manage patient risks effectively.
  • Provide ongoing patient and family support, particularly for complex cases.
  • Monitor patient progress through regular interactions with healthcare providers and patients.
  • Promote treatment adherence and encourage behavioural changes for better disease management.
  • Coordinate continuum of care, including social support services.
  • Assist patients in navigating additional healthcare benefits and services.
  • Manage conflict resolution and patient-care issues to enhance patient experience.
  • Support renal patients in their treatment journey, including dialysis coordination, chronic disease management, and post-discharge monitoring.
  1. Financial Risk Management
  • Identify deviations from clinical pathways, including surgical and renal care pathways.
  • Manage financial risks related to pre-authorizations, re-authorizations, and clinical coding.
  • Support the Bill Auditor in resolving financial disputes related to patient care.
  • Track and report on cost of treatment episodes, identifying cost-saving opportunities.
  • Analyse trends and deviations to improve financial and clinical efficiency.
  1. Coordination of Care & Multidisciplinary Team Support
  • Maintain active communication with clinicians, hospitals, family members, counsellors, and admin staff.
  • Assist in documenting clinical information and maintaining accurate patient records.
  • Build and maintain relationships with external healthcare providers, including renal specialists and nephrologists.
  • Facilitate MDT meetings, preparing patient notes, test results, and relevant records.
  • Ensure timely follow-ups on diagnostic tests, specialist consultations, and treatment plans.
  • Track patient progress across clinical care pathways, including renal and post-surgical care.
  • Educate patients on treatment options, empowering them to make informed healthcare decisions.
  • Provide emotional support to patients and families navigating complex medical conditions.
  1. Clinical Process Improvement & Development
  • Engage with clinical data to enhance care coordination processes.
  • Support development of templates and workflow improvements for better patient outcomes.
  • Contribute to quality improvement initiatives, ensuring optimal care pathways for renal, surgical, and chronic disease patients.
  • Work closely with healthcare teams to enhance the integration of care across specialties.

Ideal Candidate:

  • Clinical background in nursing, clinical associates, or related healthcare fields.
  • Strong patient advocacy and care coordination experience.
  • Ability to navigate renal care pathways and support multidisciplinary care teams.
  • Excellent communication, problem-solving, and organizational skills.
  • Experience with medical coding, authorization processes, and financial risk management in healthcare settings.
Requirements
  • Knowledge of hospital patient services and billing processes
  • Knowledge and understanding of CPT/CCSA & ICD coding advantage
  • Knowledge of funder contracts, scheme rules, exclusions and benefits advantage
  • Knowledge or clear understanding of hospital industry and practices
  • Computer proficiency
