  • Medical aid patients should check with their medical aid / scheme prior to admission to obtain pre-authorisation, if deemed necessary by the medical aid / scheme.
  • Patients have to present their medical aid card and identity document on admission. Persons under the age of 18 have to be accompanied by a parent or legal guardian.
  • Patients should also bring any x-rays and medication they are currently using as well as personal clothing and toiletries.
  • To speed up admission, a pre-admission form is available from the resident doctor`s rooms and the clinic’s reception, and should be either handed through to the hospital`s reception or faxed through to us (011 362-4474) at least two days prior to hospitalisation.
  • Also refer to the “Tariffs” and “Discharge” sections below, for more information on pre-payment and payment procedures.

Pre-Admission and admission

We have a pre-admission clinic that must be visited before admission into the hospital. Here we will obtain authorisation from your Funder, complete all your documentation and take your medical history. This will ensure that you have a smooth admission without any delays. If you cannot visit the pre-admission clinic before your scheduled operation, forms can be collected and faxed through to reception on 011 362 4474.

The following items are required for admission:

  • Current medical aid membership card
  • Identification document
  • Pre-authorisation number, if applicable
  • X-rays, if applicable
  • Medication, if in use

  • Please note that the hospital has 24 hour surveillance cameras at strategic areas.
  • We do not take responsibility for the loss of personal belongings e.g. Cell phones and jewellery etc. Please leave valuables at home to prevent loss.

Service monitoring

In keeping with our goal to continue providing high quality care; we monitor our service to patients on an ongoing basis. On discharge patients are asked to complete a questionnaire regarding their stay at the hospital. Participation is voluntary, but this will assist the hospital management to address any problems and improve our service to you.

We especially welcome any suggestions our patients may have. If you have any suggestions for us, please send us an e-mail at marika.vanderlinde@lifehealthcare.co.za.

Visiting hours

We welcome all visitors, as we know what a difference the support of close family and friends can make to the health and well-being of patients.
